See Care Coordination In Action

Ohio's seven Medicaid Care Coordination Plans currently serve 1.5 million low-income Ohioans on Medicaid. Every consumer enrolled in these Medicaid Care Coordination plans are part of a Health Care Home approach to receiving care.

This model has provided a $1 billion fiscal benefit to the state's budget. It's also helped countless Ohioans live a better quality of life. Each of the seven health plans serving Ohio Medicaid enrollees is the Health Care Home for its members. They partner through the Ohio Association of Health Plans (OAHP) to focus on the mission of the Health Care Home: comprehensive caring that helps Ohioans stay healthier, heal quicker, and take control of their own well-being. And that helps keep the cost of health care in control for Ohio. The mission of the Ohio Association of Health Plans is to promote and advocate quality health care and access to a variety of affordable health care benefits for all Ohioans.
